Before Registering You Will Need the Following Information & Completed Documents:
  • Copy of Birth Certificate for upload (New Players Only)
  • Copy/Screenshot of Skyward Proof of Grade or Last Report Card to upload
  • Insurance Information to complete form
  • THE FORMS BELOW DO NOT EMAIL! YOU WILL CHECK A BOX DURING REGISTRATION FOR THESE! (WITH THE EXCEPTION OF MYFL PLAYER CARD FOR TACKLE AND FLAG FOOTBALL)
  • Anti-Bullying Contract (Download Blank Form)
  • Code of Conduct (Download Blank Form)
  • Amateur Minor Athletic Waiver and Release of Liability (Download Blank Form)
  • Heads Up Concussion (Download Blank Form)
  • Medical Treatment Consent (Download Blank Form)
  • Metro Youth Tackle Football League Registration Card (Download Blank Form *This is for Tackle Football & Flag Football Only!) (THIS FORM YOU WILL EMAIL TO pinckneyfamily12@yahoo.com) THIS IS THE ONLY FORM TO BE EMAILED!
  • 2nd Grader Play Up to Tackle *Approved* Application (Download Blank Application *This is for 2nd Graders wishing to play Tackle Football & Tackle Cheer. Playing up must be emailed to hollyyouthfootball@gmail.com and approved by the Board of Directors prior to registration.
  • Volunteering for Stampede Duties is expected. For more info, click here.
